APPLICATION
INSTRUCTIONS
The
deadline for submitting a completed application is May
1, 2004. Scholarships will be awarded to nursing students
enrolled in associate degree, baccalaureate, master’s,
or doctoral programs.
School
criteria:

Enrolled
in a California school of nursing. |
| •
|
Grade point average of 3.0 or greater--must submit
documentation (official transcript). |
| •
|
Completed the following units of nursing courses: |

15
semester units of nursing courses or |
| |
23
quarter units of nursing courses. |
Application
must include:
| 1.

Completed
application form |
| 2.
|
An
official transcript |
| 3.
|
A
one to two-page typewritten essay documenting the
need and reason(s) the student considers him/herself
to be a good candidate for the scholarship. Include
leadership activities, financial need and other
contributing personal qualities. |
| 4.
|
Two
letters of reference, at least one of which is from
a nursing faculty member where the student is currently
enrolled. |
Submit
the packet of information (completed application form,
official transcript, essay, letters of reference) to
be received at the address below by May 1, 2004.
